•Vagina vault is a common site of recurrence in endometrial cancers though the rates are markedly reduced with the use of adjuvant vault brachytherapy.

•Vagina vault recurrences which are not amenable to surgery can be treated with transrectal ultrasound-guided interstitial brachytherapy.
